Ma-Ri sees In-Wook off and gets a message from him who asks for dinner. But she vomits in the bathroom. She remembers he kissed her neck and cuts off her hair while crying. She spots the note that he likes long hair and wants to see it at dinner. But she ignores it.
In-Wook waits for Ma-Ri in the restaurant, but his man tell him that he cannot find his wife. In-Wook asks for the CCTV footage and spots Ma-Ri running away from home. He hits the table and leaves. Ma-Ri waits for Jung-Won in the rain. Sung-Ho and Hae-Ra invite Jung-Won to dinner, but Jung-Won rejects it.
Ma-Ri follows Jung-Won to home but she passes out. In-Wook returns home with his man and finds the note and jewelry which his wife left. He breaks the jewelry and asks his man to find her. Jung-Won gets the blanket on Ma-Ri at his house and gets the clothes for her. He tells her that it belongs to his wife.
She takes off her clothes and shows the wounds to him. She says that she cannot live with the face and asks him to change it for her. In-Wook spots Ma-Ri’s hair in the bathroom and breaks the mirror with the scissors.
Ma-Ri tells Jung-Won that In-Wook was a good guy at first, and she wanted to leave her life to him. She says, “I met him for the first time during my graduation exhibition. I found out later, my face in my painting looked like his mother. It’s not that he wants me. He only wants to see me looking like his mother. So he never hits my face. It’s his precious thing that shouldn’t be ruined. My misery started with me face.”
Jung-Won tells Ma-Ri to call police, but she says that she ended up getting captured again every time because he doesn’t care if it’s legal or illegal. He says that he cannot perform a surgery like that. She asks him if he has ever done anything he regretted.
In-Wook place a ring on the table, and it surprises Ma-Ri. She says that it’s been less than a month since they met. He takes the ring and claims it’s his first time to get down on his knees, and asks to get married in a month. But she says that there will be an open exhibition in a month, and she needs to prepare. He says that he wanted to marry her when he met her, and asks her to take the ring because it belonged to his mom.
She asks him why it’s her since there are a lot of women who suit him better. He says that those women aren’t her. She tells him that she cannot get married because her mother is sick. He saya that alcoholic dementia cannot be treated at home, and offers to take her to the sanatorium. She is surprised that he knows her mother and accepts the ring.
The man walks In-Wook and tells him that they found out Ma-Ri’s whereabouts after checking the CCTVs. Ma-Ri says that she wants to go back to before she was married, and asks Jung-Won to change her face. She says that she will kill herself. Hae-Ra knocks at Jung-Won’s door and asks him why he didn’t answer her calls. She tells him that they have to go to hospital, and reveals Sung-Ho ran a problem during surgery. He tells her to go first and informs Ma-Ri that he has to leave.
Jung-Won arrives. The assistant says that he warned Sung-Ho not to pushed ahead with the surgery. Sung-Ho talks back that the woman kept insisting. Jung-Won stops their argument. Ma-Ri wakes up by the nightmare and remembers In-Wook told her that her mother is with him. So she goes outside and calls the sanitarium. The woman tells her that her mother is getting worse all of a sudden. In-Wook tells the woman that she did a great job.
Jung-Won returns home but Ma-Ri left. Ma-Ri visits her mother but spots In-Wook. Jung-Won arrives at the hospital. The doctor tells him that the patient’s vital signs are stable.
Love in Sadness Korean Drama Recap: Episode 4
Hae-Ra tells Jung-Won not to get involved when she knows Ma-Ri asked him to change her face. She offers to find a skilled attorney for Ma-Ri. In-Wook asks Ma-Ri what’s wrong with being greedy over you and holding onto his love since that’s how everyone lives. Ma-Ri yells that they cherish and adore the other’s family.
He asks her to live for his sake, but she thinks he is like a ticking time bomb. She says that she is scared and frightened. Jung-Won remembers Ma-Ri’s wounds and wonders if she is all right. Ma-Ri stays in the bathtub and looks at herself through the mirror. She remembers In-Wook thought that she will dump him and ran away when he looked at her eyes. She washes the mirror.
Jung-Won looks at Ha-Kyung and remembers he gave her the wedding anniversary gift. She thought it’s pretty, so he wore the necklace for her. He asked her how long will her business trip take. She said that it’s a week, and claimed it’s the client’s schedule. He wanted to drive her when she was going to leave, but she refused it. He held her hand and said that he had a strange feeling, but he had to release her hand.

Jung-Won saw Ha-Kyung off outside, and In-Wook just walked Ma-Ri. The kissed Ha-Kyung’s hand in the car when she was driving. She laughed but had the car accident. Jung-Won was informed about the case and arrived at the emergency medical center. He claimed he is a doctor and wanted to operate on his wife. But the doctor stopped him.
Hae-Ra tells Jung-Won that the artist is unable to continue her career due to personal reasons, and she’s out of touch. He asks her to see Ma-Ri on his behalf if she visits. He tells her to allow Ma-Ri staying at his house. But Hae-Ra is against it.
Il-Kook hunts with In-Wook and tells him to blow the other’s head off first if he doesn’t want to die. He says that if anyone tries to betray him, teach them a lesson. He thinks In-Wook is indeed his son after he shot the bird.
Ma-Ri is startled when In-Wook holds a gun and targets her. He tells her that it’s not loaded. But he loads it, and shares that his father killed his mother because she tried to run away. He asked his father why he let her die that way. Il-Kook said, “because she’s mine.”
In-Wook says that he came to understand his father when he met her. He asks her to hold the gun and point at him.
